Student Employment Forms

The forms on this page were created in Adobe® portable document format. Some forms can be completed online, and printed for submission to Student Employment. Some forms must be completed manually.  Forms can be downloaded using Adobe® Acrobat Reader®.

There are several student employment employee types.  Each employee must complete the forms or combination of forms appropriate for their employment type.  The types of employment are divided into subsections on this page.  In addition, certain items of personal information require other forms. 

Some students may be required to complete forms from several sections.  Carefully review all employment types and forms on this page to create a complete student employment packet.

First Time Workers

A student who has not worked previously in a student employment position must complete all of the forms listed in this section and submit to Student Employment before he or she begins working. Both the student employee and the supervisor must complete the Student Employment Authorization form and contract. These forms must be submitted to Student Employment before the student begins to work.

The individual who completes section II of the I-9 form must see original documents and include a photocopy of the original documents.

The Federal government provides a list of acceptable documents needed for completing the I-9 form. For a list of acceptable documents, opening in a separate window, refer to page four of the I-9 online document:

International Students

An international student who has not previously worked in a student employment position must complete all of the forms listed in this section, as well as the in the First Time Workers section. Both the student employee and the supervisor must complete the Student Employment Authorization form.

In addition, an international student must have a Social Security Card or receipt of application for the Social Security Card, and a copy of their email from University Center for International Education, verifying their Work Authorization. An international student must also bring his or her original Passport, form I-94, and form I-20. All forms must be submitted to Student Employment before the student begins working.

An international student who has not worked for 30 days or more in a student employment position must also submit to Student Employment a copy of the email, from University Center for International Education, verifying their Work Authorization.

Previously Employed in a Student Employment Position

A student that has worked previously in any student employment position must complete and submit only the forms listed in this section. Both the student employee and the supervisor must complete the Student Employment Authorization form. This form must be submitted to Student Employment before the student begins working.

If the student has had a change in name, address, bank account or tax information, the student can complete the change of information forms found in Other Forms.

An international student who has not worked for 30 days or more in a student employment position must also submit to Student Employment a copy of the email, from University Center for International Education, verifying their Work Authorization.
