Cooperative Education and Internship Exit InterviewsUpcoming Exit Interview dates and times Exit interview requirements apply to all students registered through Career Services for a field-related work experience. Students in cooperative education or internship positions are registered for CPE 091 or CPE 092. Students working in cooperative education or internship positions with a Liberal Arts Learning Contract are registered for one of the following: LA 203, LA 205, LA 303, LA 305, LA 403, LA 405. Students who participate in cooperative education or internship programs are required to complete an exit interview in Career Services, during the last two weeks of the quarter in which they are working. Things to Bring to the Exit InterviewThe student must make the following items available prior to or during the exit interview: A performance appraisal completed and signed by the supervisor. The supervisor will be emailed the performance appraisal by the Career Services staff. Career Services expects that the supervisor will review the appraisal with the student prior to the exit interview. Students who do not receive such a review should request one. A type-written or word-processed job summary highlighting duties and accomplishments, one page minimum in length, will be turned in during the exit interview. Students registering their co-op or internship for the upcoming quarter may request a co-op acceptance form prior to the exit interview. All contact information, including job title, wage, supervisor name, supervisor title, supervisor's email address, company name, complete company mailing address, company area code and phone, area code and FAX number, and company web page address, and hourly wage will need to be updated and /or completed. If you would like to share information about your co-op experience with other students, please bring a digital photo of yourself in the work place, as well as the company logo. Your Career Services liaison will ask you to complete the Co-op Testimonial and Photo Release forms. Additional Information for Liberal Arts Learning ContractsComplete the exit interview requirements listed in the previous paragraphs. In addition, students must turn in their assigned academic project and meet with their faculty mentor prior to the last day of classes. Subsequently, the Liberal Arts Learning Contract faculty mentor will forward a grade recommendation to the Career Services adviser. Instead of a job summary, Liberal Arts Learning Contract students should bring a copy of the paper or any written project assigned by the faculty mentor. Exit Interview ScheduleTo schedule exit interview appointments in advance, please contact Career Services. The front desk has access to adviser's schedules. It is not necessary to speak to the adviser to schedule the appointment. Any student who does not complete an exit interview will receive an INCOMPLETE on their transcript.
